Good points throughout; I especially appreciate the author's note that a single good exam question can take 30 minutes or more to write. This has been my experience as well; a good question should be one that doesn't reward guessing. That is, while there is one right answer (or more for multiple-answer questions), all the wrong answers are plausible as well such that the student really needs to know the topic or content in order to get it right.
